You may have had a telephone consultation or be due to have one and been asked by your doctor or member of the reception team to send in a photo of the problem. Our clinicians ask that you send one image directly facing your complaint, and if necessary one image from the side.  

We ask that you please:

• Check the image is not blurry
• Attach your name and date of birth to your email
• Do not send in any unsolicited or inappropriate images

After submitting your images, please call the practice after 30mins to confirm receipt of the images, and to check they have been placed into your notes.
